MLFA ALL ACCESS FIELD TRIP PERMISSION SLIP

Dear Media, Law and Fine Arts Families,

At The Middle School of Media, Law, and Fine Arts, we believe that field trips, cultural outings, sports competitions, and other school related trips that take place outside of the classroom are not only an important part of our school culture but also an integral part of developing the whole child. We are lucky to live in New York City, where there are many opportunities to visit museums, parks, performance art institutions, and much more. During the 2020-2021 school year, we plan to offer all students the opportunity to attend various field trips that will reinforce what they study and engage in within the classroom, making real world connections between the class and the community. These trips are meant to provide a meaningful learning experience to each student and may take place either during the school day or after school. Depending on your child’s participation in school sport’s teams or other clubs, some trips may even occur on the weekends. Additionally, some trips may require an adult to pick the student up from a location off-campus. If your child is invited to attend any of the above mentioned trips, you will receive a notification flyer detailing the date and location of the trip. In certain cases, a student’s behavior may warrant a parent chaperone. In the event that your child requires a chaperone for a trip, the student’s guardian will be notified accordingly. In the event that you do not want your child to attend a particular field trip, you can do so by completing a “Singular Trip Opt-Out Form.” The “Opt-Out” form must be completed and returned to the school at least 24 hours prior to the trip. Upon return of the “Opt Out form” it is also expected that you will contact your child’s Advisor to inform them of your “Opt Out” request.

To provide consent for your child to participate in all school related trips for the 2019-2020 school year, please fill in the blanks with the appropriate information, sign, date and return this completed Consent Form to your child’s Advisor.

In consideration for my child being able to participate in these school related trips, I agree to release my child’s medical information and provide permission for MLFA Staff and all affiliates to act or make decisions on my behalf in the event of an emergency.
